Signing Waivers, Consent Forms, etc.

Before signing any forms whether you are at a doctors office, a vet's office or any

other facility or professional office, although tedious, you need to read the form

and add any language to clarify an issue that might come up and cross out any language that you feel uncomfortable with or disagree with. It is important to do

this because if something happens to you, a family member, pet or a friend, the

facility you are at will try to use their Waiver or Consent form against you. .. . They

won't be able to if you make sure the form states your agreement and the form could

help you prove a case against them if you pursue a claim.
